One thing that will never cease to unnerve me in Skyrim (and many other games by the same studio) is how some suits of armour will radically change depending on the gender of the wearer.
You can loot a suit of armour off a body of the opposite gender and it will change the design (such as adding boob plates or swapping pants for a skirt).
I understand the gameplay requires the armour to magically be a one-size-fits-all but is it really necessary to make it completely re-design itself?
